Hemorrhoids In Rectum. Prescribed Cream. When Will Pain Subside?

Recently realized a have hemmorids. Self diagnose via Wikipedia. Went to doctor today to confirm my findings and seek treatment. The doctor did not even examine me, just prescribed some cream and sent me on my way. The hemeroid is sticking out of my rectum and is very uncomfortable. Can't even get the cream tube inside to apply cream without screaming. When do I panic?

Gastroenterologist 's  Response
Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM).

Painless fresh bleed per rectum may be hemorrhoids
Painful bleed per rectum is more likely to be anal fissure

So I think you are having anal fissure alone or along with hemorrhoids

What all you have to do is:
1. make your stool soft either by taking lots of fibres (psyllium fibres) if not helpful then add laxative also to soften your stool.
2. Don't strain while defecation
3. Sitz bath at least 4 times a day
4. Take ointment containing diltizem that has to be applied at anal verge instead of putting inside with help of finger before and after defecation

I hope I have answered your query and this will help you. Remain in touch and get-well soon.
