Hi, I'm a 22 y.o. female. I've grown up playing tennis and recently I've had L shoulder instability. It is sitting anteriorly, and clicks when I move in most directions. Some of the clicking comes from the shoulder itself, but a lot of the clicking actually comes from my scapula. Also, about as long as my shoulder instability has been occurring, I've also had nagging p! just inferior to my L inferior angle. That pain has been worse the last few weeks, and more constant. It is hard to tell, but feels like there is a slight bulge on the lateral portion of my ribs in that regions (but I have no rib pain anteriorly around my abdominal region, just posteriorly). I've been assuming that it's just my ribs subluxing, but when I get cracked by my PT (who I have been seeing now for 2 months for my shoulder instability) that spot never cracks nor gets relief like the rest of my t spine. [I should probably mention I'm a graduate PT student]. I have now been to 2 separate orthos who focus on my shoulder joint itself and not my scapular pain and clicking, when these days that is causing me more pain and discomfort than the shoulder joint itself. PT has helped me increase my ROM, but I'm just still unstable. At my last appt last week my shoulder got stuck in an abducted and slightly IR position after doing IR with the thera bands. I got it unstuck with ER, which was painful. I've also, just in the last few weeks had random sharp pain to my L elbow randomly. Since then it has been catching and locking up like that fairly regularly. Additionally, since all of this has been going on my t spine feels like it needs to be cracked constantly. When I don't have PT my husband cracks my back, and it is getting to the point where he'll do it 10 times in one night. Also he'll compress on my ribs and scap, it is painful, but also makes it feel better at the same time... . Anyways I would love to avoid any surgery for now since I am just trying to get through my grad school, but am getting to the point where I just want this to feel better. I feel like I've reached a dead end with the orthos as they seem dumb founded by my scapular situation, and I love my PT but it is just providing temporarily relief. I've been doing scap stab exercises up the wild zoo and it doesn't seem to be doing much. I've had 2 x-rays since June. The last one being in December and they both were clean, no bony bankhart. The one doc suggested an arthrogram, but I had lousy insurance at the time so I didn't have it done. Any suggestions or ideas of what could be causing my scapular/rib pain and clicking.
