Hi I'm Having Extremely Painful Periods With Very Minimal Bleeding

Hi I'm having extremely painful periods with very minimal bleeding this month , however past months had extremely heavy periods and minimal pain ... Even getting double period last month
I'm 39 and have 1/2 thyroid due to a sudden thyroid heamorage and atypical cells were found , I have been told it's only a matter of time until I lose the rest of my thyroid ...
I haven't had a Pap smear in 11 years either and have Ehlers Danlos Syndrome type 3 & orthostatic hypotension .
My thyroid has been playing up again ( what's left of it ) & I question if this is throwing my periods off ?
Even heat is not easing the cramps ...they are normally regular & I always getting a migraine the day before they start too ...
Just wondering why so painful yet it's so much lighter then normal ?
