How UTI Is Treated Without Knowing The Bacteria?

My father diagnosed with malignant bladder tumor 2 years ago.; He has been hospitalized last 2 weekends for 4 days and was told he had UTI. Cultures were done with no growths on both weekends. He was treated with IV antibiotics and sent home on oral antibiotics.

I am not understanding how they know which antibiotic to treat with if they do not know the bacteria Thank you, Vicki

* Most common causative organisms are E.coli(50%),
gram -ve species ( Klebsiella, proteus, enterobacter , pseudomonas (40%)
grm +ve ( E.faecalis, S.saprophyticus, S. aureus ) - 10%
Thus clinical treatment starts and target most common susceptible organisms
even though culture is negative.
Best clinicians always treat patient on his judgement rather than relying
totally on mechanical parameters.
