My nephew is a 22 yr old with a strong family history of coronary disease, and himself on a statin for high cholesterol. He is overweight, but exercises hard and often. He is doing engineering, and finds the course very challenging. He reports a 10 minute long episode in class yesterday during which he had chest pain, anxiety and sweating. On questioning he says the pain was crushing in type. He left the class and in minutes felt better. Could this be a small cardiac event eg a transient thrombosis? Should he have an EKG at this point? Something more invasive? (Both grandfathers died of cardiac events, one at aged 38. His father is well).
