General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

We will try to help you in best way possible. First of all it's sad to know what you had been going through lately. Based on the history you have shared, you will need to continue anticoagulant therapy for six months or depending upon your neurologist advice. You need to increase the dose to keep INR atleast 2 and preferably above 2.5 and strict follow up is a must. Warning signs like bleeding or black stools etc should be cautiously monitored. Life long folic acid treatment might be needed as well.

Hope I have answered your query. Let me know if I can assist you further.
