welcome to HCM!
I have read your question carefully and from that it looks that you have
anxiety attacke, panic attacks in which you have derealization ( which is one of the feature of panic attack). You are not on any treatment for the same and want help.
Firstly, I want to tell you that panic attacks are completely manageable and most of the patients become 100% good and also able to stop the treatment after some time.
You have not been to any doctor ( at least it is not mentioned in question) for the same, and duration if problem is not mentioned.
the only thing of concern is you black out when you stand up? This is not a feature of panic attack, it is not related to standing up and having lightheaded senses / black outs?
you should get your blood pressure checked and get some blood tests, may be a CAT scan of brain to rule out other causes.
For panic attack, I have seen excellent response with
paroxetine, may be in a dose of 12.5-50 mg in a day. This will reduce the anxiety and stop attacks with in few days.
For acute panic attacks, I usually recommend
clonazepam 0.25-0.5 mg SOS.
So, for your
lightheadedness and
blackout which you have while standing up, please do visit a doctor. Rest, I think panic attack can be managed as above in most cases.
Get well soon
