How Can Be Done To Make The Kidneys Strong Enough For Better ARV?

My father is 60years old, weighs 49kg and is HIV positive. He has been on ARV's for 2 years now and it was improving his health. Lately he has been been losing weight and feeling very tired. He went for blood tests and his Viral load is increasing and is high (between 20k and 30k) but he cannot change his ARV's because his kidneys are not strong enough. Should he change his ARV? How can he make his kidneys strong enough for a better ARV?

Only two ARVs are to be avoided in HIV patients with kidney diseases --Tenofovir and Indinavir . Other drugs whose dosage is to be curtailed in these patients are Didinosine Stavudine, Lamuvidine, Emitricitavine,and Maraviroc .
So I am not sure what combination of ART , your father is taking . So I suggest that if he is on Tenofovir or Indiavir , these medicines are to be substituted as per resistance testing. Rest other medicines dosage are to curtailed .The matter of concern is his increasing VL, which is to be taken care off. So my advice -take hom to a local HIV physician and he will set the medicines and kidneys will be saved from further deterioration.
