How do you differentiate the cause of goiter? I was diagnosed with goiter and the only option given was surgical. I would like to know what options are available other than surgery and what tests need to be done to determine if surgery is really the only option.

The most common cause of goitre is iodine deficiency. If needed iodine supplements can be prescribed for that. Also, check serum TSH and free T4 level. Hypothyroidism and hyperthyroidism needs to be treated accordingly with medication if needed. Rule out hashimoto thyroiditis by antimicrosomal antibody testing and Graves’ disease by anti TSH receptor antibody.
For extreme thyroid enlargement, partial or full thyroidectomy is the option. Consult surgeon for examination.
