How Can Hypertension Be Treated?

I have been struggling with high blood pressure for about 3 weeks. My PCP increased my amlodipine up to 10 mg (I think BID), I was also out of my tramadol and it has been reinstated. My diastolic pressure has been the most worrisome as it has gone up to 127 this past weekend. It was 111 this morning. My BP tonight was 139/76, which is the lowest I have had in a long time. I have had a headache through this time period. I have multiple health problems; end stage pulmonary fibrosis, one kidney and MS. My blood tests on Thursday showed a decline in my kidney function combined with an increase in white blood cells. We are hoping it is just a sinus infection. My question is that it feels like the posterior of my neck is swelling slightly, Is this something to be concerned about at this time?

General & Family Physician 's  Response
Hello,

According to my opinion, your high blood pressure is due to Renovascular hypertension as well as mitral stenosis (MS). Besides pulmonary fibrosis also play a role in elevated blood pressure.

As far as treatment concerns, 139/76 blood pressure can be considered in the normal range. So, if Amlodipine drug twice a day is maintaining it up to this mark, then no need to worry.

Now you have to manage MS by surgical valve replacement, kidney function by diuretics or dialysis if required. For pulmonary fibrosis, please consult the pulmonary physician.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Hiren Hirpara
