How Can Itchy Hemorrhoids Be Treated?

I have been using Preparation H cream for a few months thinking I had hemorrhoids- it does help but I also think it might be a fissure.Sometimes the itching is awful!!! I have been using the medicated wipes for about 1 month. I don t know if I need to schedule an appointment with my Dr - could this be a sign of anything else?
