Scan showed inflammatory tissue reaction in both adnexae as well as in the pouch of Douglas causing poor interface between pelvic organs and a mild free collection in the pouch of Douglas. Been on antibiotics for weeks but still feeling lower abdominal pain

With what you have described in the scan, it seems that you have Pelvic Inflammatory Disease. The course of treatment is a long one. It takes up to 6 weeks to clear. Be on regular follow-up with your gynecologist, who will probably run another scan to make sure that the antibiotics have worked. Do continue and complete the full course of the anitbiotics.
