I have prostate cancer as confirmed by two previous biopsies. My PSA levels have historically been high and erratic, ranging from 3.0 to 7.0. (up and down over the past 7 years). My PSA level this past May was 5.5. My PSA test results from 4 weeks ago were 11.1. ) My Urologist of nine years (inclusive of this past may), retired in August and had recommended an MRI in the event of increased PSA levels. My New Urologist wants to do another Biopsy. What of these options are the most accurate or is there a better option? Thank You. (p.s my age is 64)

Based on the history you have shared, both MRI and biopsy should be done , as both will reveal the type of growth and extent of spread. Meanwhile close liason with a urologist should be kept, to discuss for anti androgen therapy if needed. If urinary complaints like retention etc appear catheter may also be needed to reduce urinary retention.
