How Can Severe Osteoarthritis Be Treated?

Hi, I am 65 years old and have chronic pain from osteoarthritis, arthritis, and a T8 fracture. I have been on high doses of Tramadol for 12 years. Last 2 years I decreased to 50 mg tid. My doctor is afraid to give me more than the long acting 100mg form.. This is not working. Why? YYYY@YYYY

every drug has a tendency to develop tolerance i.e its effect/response decreases with the usage.
