Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here are my thoughts.
Your son needs an urgent evaluation by a Psychiatrist as this looks to be a severe case of psychosis and in spite of medications for depression. Threatening, suicidal tendencies, imaginations as you have explained need urgent care. The pain in the foot may be felt severe due to this reason also and obviously an X-ray can tell whether there is a fracture or not. And will be treated likewise. If he has no insurance, Govt run facilities can helps him.
Hope I have answered your query. Let me know if I can assist you further.
