Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

If the pain is persistence with the history of anal fistula it is indicative of inflammation, infection, some residual tissues, foreign material inside the fistula canal, no analgesic relieving pain it means condition need to be taken under consideration for exploration, surgical intervention may needed if nothing is indicative on clinical examination.

Hope I have answered your query, let me know for further assistance.
