My baby is about 23 months he always use his hand push me to understand his demand. His ear no problem. He can say papa, duck like this not Over10 words. His adenoids gland have problem. He want to say but can’t speak out. Make some sound which I not understand. What should I do

If he is 23 months old with speaking difficulty and using sign language then it is an indications that he needs to be worked up for -
1. Isolated speech delay
2. Autistic Spectrum disorder.
3. Hearing defects.

I need to take more history to assess him.
