General Surgeon 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

As well known and must be explained by your consultant, Penicillin G is the gold standard therapy for syphilis management.

The four fold decline in TPHA titer (as in your case 160 from 680) is considered well response to the therapy.

The total negative report may take many more months say even 24 months in certain cases. So do not panic regarding the same and continue current medications with regular follow up with your consultant.

Thanks.

Regards.
