Hi,
Home oxygen therapy and NIV are the maximum that can be done in such a severity of COPD patient. How is his 2d echo?Is there any pulmonary hypertension. Unfortunately COPD is an irreversible disease except in acute exacerbation. That means the disease only progresses and we can try to slow the disease progression. Continue inhaled therapy and oral medications. 1/6th of lung capacity is poor prognosis however the exact survival is difficult to predict.
You need to continue his medications.
The only theorotical hope is bilateral lung transplant if heart is normal and if such a program exists in your area and the patient is eligible for the same.
