Hello,

As the doc has started meropenem (second line drug) along with ceftriaxone and oflox (conventional first line antibiotics), in my opinion, intensity of typhoid fever sounds to be high (though report of Widal test is a must to co relate clinically). When intensity of typhoid is mild to moderate (say Widal titre of 1:80 to 1:160 or 1:200), only first line medications are more than sufficient to cure at earliest (roughly around 10 - 14 days therapy).

When the intensity of typhoid is much high as persistent fever of high grade despite oral medications or high titre of Widal as beyond 1:240 or even in some cases 1:400 also; second line therapy comes into action and predicted time period of recovery is variable with range of 4 - 6 weeks therapy in total. Periodic evaluation of titre of Widal is the marker of best predictable recovery as the load of bacteria in Peyer`s patches is key of clinical response after 2 lines of antibiotic therapy.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
