Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
I was suffering from Bladder Cancer, so my bladder was removed and a neo bladder has been created by doctors. I am having severe pain at the right side of my abodoman, near Kidney which is not working according to the doctors. The renal function of my right kidney is 4% and Left kidney is 87%. This pain goes to my leg also. As per yesterday s urine report : My RBC is 8-10 HPF and Pus Cells in urine are 40-75 WBC/HPF Proteins present 2+ and ESR is 105 (blood). for the last more than 10 days I have lost my appetite, I do not feel like eating. All the time i feel uncomfortable with pain in right side of my abdomen and also in my right leg.
* The pain involving right flank region with urine report suggestive of severe urinary tract infection (40-75 white cells per high power field) is most likely due to possible hydronephrosis or pyelonephritis condition.
* As the right kidney function is greatly hampered, there are all high chances of getting infection in ascending way via urinary infection.
My guidelines at present
- plenty of liquids and maintenance of hydration
- control of blood sugar
- urine culture report for pathology diagnosis of micro organism responsible for the infection and accordingly decision of intravenous or oral antibiotics as per the susceptibility pattern
- avoidance of certain analgesic as diclofenac or ibuprofen which may hamper kidney function further
- additional reports of total wbc count in blood with serum creatinine and blood urea are of paramount importance.
Kindly provide further reports available with you for better correspondence ahead in this regard.
Hope I have answered your query. Let me know if I can assist you further.
Take care
Regards,
Dr. Bhagyesh V. Patel
