General Surgeon 's  Response
Hi, It is unlikely that he doesnt have testicles.
He could be having retractile testicles which go into the inguinal area at certain times and not seen.
He could have been operated for certain conditions like prostate cancer where they remove the testicles for treatment.
Having no testicles is extremely rare and the conditions I mentioned and a few more are more likely and common.
Take care,
Dr Rishi, New Delhi, India.
