How To Get Rid Of Dilated Spermatic Veins Under Testicle Post Varicocelectomy?

I had a left side varicocelectomy 3 months ago which seemed to get rid of the dilated spermatic veins but there are still significant dilated and painful veins that remain behind or under the left testicle (which I think are gubernacular veins). My Dr. did not deliver the testicle in the gold standard procedure (microscopic subinguinal). Do you think he missed these veins as a result? and if so, why would most surgeons not deliver the testis if there was a chance like mine they would miss important veins. You get 1 shot to in and get what you need done and I would think they would want to get them all.
