Igg both are positive , evening rise of temperature ,
fever with mild chills ,
night chills , headache , "bilateral basal bronchovasculaur marking prominent " all are suggestive of
pulmonary tuberculosis .
Since you mentioned that sputum don't produce in the morning , take an evening sample or if possible take a cup cover it with a lid and nos spit in it for 24 hours and then get in sputum for A F B . This is novel way of sputum collection.
Any how if no sputum get in
BRONCHOSCOPY .
Consider in high risk patients ( bronchoscopy ) who are unable to produce sputum spontaneously or by induction.
Get in also polymerase chain reaction and tuberculin test for confirmation.
Sometimes when patient is not able to produces sputum Nasogastric tube passes and sample to be taken .
Since tuberculosis is a highly contagious disease , spreading through droplet infection ( by sputum droplets when patient inhale or exhale or speak to some one or
cough or
sneezing ).
So cover mouth with mask or clothes . Don't use any article of patients 9 belongings like bed sheet , pillow, cover , quilts , towels . Don'e share food items with any one directly in a plate .
Take proper treatment as advised your treating physician , never stop treatment at any cost before completion .
