Hello Your father is 77 years old and he is taking insulin, twice in day and having pacemaker . Now his serum creatinine level is elevated , so also blood urea . He is also having fluid in the lungs . Fluid also present in stomach . All these signs and symptoms may be due to these possibilities in such cases , these include: 1 Congestive heart failure . 2 Chronic renal failure . 3 Hypo proteineaemia . Frankly speaking it is very difficult to suggest you any treatment without physical examination with such symptoms . So in my opinion consult an UROLOGIST and get his opinion because physical examination as well as clinical examination and history of illness is important. Good luck.
