How To Treat A Swollen Painful Ankle Post Injury?

My son had a football injury 4 days ago. He got a stud on the outside of his ankle bone. We went to the hospital and no break but not possible to ascertain if bone cracked or a fracture. His ankle was not twisted during the accident. He is still in a lot of pain and his ankle is still very swollen. It is now starting to itch. Do I need to take him back to the hospital?
