How To Treat Body Pain And Numbness In Arms And Legs While Undergoing Radiation For Breast Cancer?

Hello, I am 51 years old, stage 4 breast cancer in 4 areas of breast and chest wall.H2positive. I had 6 rounds of chemo and 25 doses of radiation. I had 29 lymp nodes removed 10 were positive for cancer including the last one the doctor removed. I have a strong family history of Breast C, I lost my mom from Breast to Bone over a span of 7yrs. After my treatment I took Tamoxafin for 5 years, then Femara for almost 8mos. the side effects were so overcoming I was changed to co-exemestane last August. I am feeling like I have aged so much. The pain in my body is so overwhelming, I can t even enjoy the simple things like walking my dog or going for a ride on my horse. The pain is incredible. I have acute numbness in both my arms and hand, I have 7lb strength in my right, my left arm has lymphedema and nobody will help with the numbness on that side. Its too risky they say. It seems the longer I take it the worse the effects are becoming. My question is. Is there any other option? What are the long term side effects of the drug and can the symptoms be effectively managed so I can continue to work. I was put on Cymbalta to assist with pain management and for coping. Its not working very well. I am sensitive to a lot of medications and cannot take anything in the daytime that would effect my cognitive ability, I drive a lot with my job.I have honestly been considering just stopping the drug. My quality of life is not there, I am getting so tired of being unwell. when I was such a happy and active mom. I am told I am about a 95% risk of Bone Cancer if I don t take the Medication. This isn t something I take lightly as I live for my children who are 18 and 20. But trying to get Doctors and help on this Island is a challenge in itself.I am wondering if you could look at what I have sent you and perhaps you have Ideas or suggestions. My family Dr is really great to try anything for me. Thank you so much Marianne D

Anesthesiologist 's  Response
Hello,

First of all, I appreciate your courage and attitude towards Life and your fight against breast cancer. Common adverse effects of Exemestane are as follows:
>10%
Fatigue (22%)
Nausea (18%)
Hot flashes (13%)
Depression (13%)
Pain (13%)
Insomnia (11%)

Thus, pain is one of the side effects of Exemestane. The following are recommended in treatment of arthralgias or joint pains induced by Exemestane:
1) Physiotherapy
2) Massage therapy
3) Mild to moderate pain: Acetaminophen, Ibuprofen, Tylenol with Codeine, Tramadol
4) Severe pain: combination of Cox 2 inhibitor, NSAID and opioid like Tramacet + Percocet + Tylenol
5) Pain modifiers like Gabapentin can also be used
6) Lifestyle modification:
* Daily exercise
* Morning warm up
* Reduction of weight if obese
* Eating only healthy foods like veggies, fruits and nuts, fish, egg etc.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Pallavi M., Anesthesiologist
