hi, i just had my baby 3 weeks ago, i am passed the engorgement stage but now my milk supply seems to be low and my breasts feel sore to the touch, although i have no signs of red or plugged ducts. im strictly breastfeeding and doing it every 3 hours or so. i also feel like im about to get a lung infection, my body aches and im coughing up nasty phlegm.

Dear Mumma,

You are doing the best thing of your life; i.e. breastfeeding your child.

Usually, the milk production is not low in quantity if the breasts are emptied every feed.

Now understand the physiology. The milk is produced under the stimulating effect of a hormone called as Prolactin. The ascini in the breasts get filled with the milk that is produced by the glands. As they go on filling, they give negative feed back to the pituitary for production of Prolactin, thus the milk production is controlled. When baby sucks the milk, the ascini get emptied giving positive stimulus for production of more milk. So the bottom line is that the breasts should become empty after each feed. If some milk is remaining, squeeze it out and store. You can utilize it for giving it to the baby later. It can be stored at room temperature for 8 hours and for 24 hours in freeze.

The other cause of low milk production is faulty technique of suckling. Most importantly, please DO NOT GIVE ANY PRELACTEAL FEEDS LIKE HONEY, SUGAR ETC TO THE BABY. It kills the baby's hunger and baby does not suck the milk to the fullest requirement. For sucking the breast, baby should not suck only the nipples. By that, it has to use extra force, gets fatigued earlier and leaves the nipples early leaving the ascini half emptied. Babies lips should go round the areola and nipple touching the palate. By this it just squeezes the nipple by tongue and gets milk without much physical work. By this technique, complete breast gets emptied, giving stimulation for more milk formation. If the breasts do not get completely emptied, please squeeze out the remaining milk for more and better milk production.

After suckling on one side, before taking to the other breast, please hold the baby erect on your shoulder, pat on its back and burp the baby so that the air goes out. Trapped air in the stomach fills babies stomach and baby cannot drink more milk.

After the baby is satiated with milk, it will sleep and leave the breasts of its own. Please do not pull the baby away from the breasts while the nipple in its mouth.

There is lot more to be explained on breast feeding, however, there is limit of the space.
