With the history you have provided I would advise you the following:
Consult an Orthopedic Surgeon for proper clinical evaluation, examination and investigations like
MRI scan of the affected area so that any associated problem related to discs, nerves can also be detected along with problems in bony structures.
A course of appropriate anti-inflammatory medicines with painkiller property.
Sitting in a warm tub.
Muscle relaxants if there is associated muscle spasm.
Oral
steroids if no
diabetes.
Surgery is usually not needed if the medical conservative management solves your problems.
Surgery is done only if indicated.
Take laxatives to have soft bowel movements.
