I Am A 50 Yr Old Female With Diabetes Since

I am a 50 yr old female with Diabetes since 1998. I am on a variety of meds including insulin. My father is also diabetic and he has had triple bypass surgery 12 yrs ago. My question is this - I have never had problems with my feet but over the last 6 months, I have found that my big toe is quite often blue and very cold and/or numb. I also have much pain in both my arms and shoulders and pain across the middle of my back. I have also had very little energy and heaviness in my chest. I am wondering if these signs could be heart issues/circulation problems. What are your recommendations? What steps should I take next?
