I Am Currently Taking Perphenazine At 16 Mg Which I

I am currently taking perphenazine at 16 mg which I have been on for 26 years. For the past five years I have also been taking lexapro 5mg and topamax 37.5mg daily all for bipolar 1. I feel at many times over drugged and angry/agitated and really tired on this combination of meds and tried a few times on my own to wean off of both topamax and escitalopram but things ended up worse than before when I did this. I also take levothyroxine for hashimottos. I have been told by a few doctors in the past that the dosage of perphenazine I am on is quite high. I'm now thinking about very slowly reducing the high dosage of perphenazine to at least 4 mg's less than I am currently taking so I will not feel so tired during the day and will be able to concentrate better as my 2 jobs require, and remain on my normal dosages of the other meds . Does this sound reasonable or do you have any other suggestions?
