I Have A Spot On My Scalp That Has Been

I have a spot on my scalp that has been there for about 2 years. It will scab over & then it never heals. It will bleed if I pick at it or when I wash my hair. There is no pain. Occasionally it will itch. I am also experiencing hair loss all over the scalp all the time. It's worse when my hair gets dirty. Don't know if the two are related.
