I Have Admitted A Patient Having High Grade Fever With

I have admitted a patient having high grade fever with chills,there were history of sever pain in abdomen 2days before I have done usg abdomen which shows cholecystitis with cholilithiasis TLC IS 15000 polymorph 83%seen by general surgeon he says clinically there is no such pain & tenderness as seen in acute cholecystitis,there may be some other reason for fever but I have done malaria scrub typhus laptospira which are negative. Jaundice is present but SGPT is normal . . bilirubin is 4.4 & direct is 2.4
Rota virus is negative
fever recurrent every 6hours
what will be the cause?
