I Have An Itchy, Red Skin Irritation On My Left

I have an itchy, red skin irritation on my left ankle that has been there most of the time for the better part of 1.5 years. It can improve & appear to be healing for a short time, but then get terribly itchy which causes scratching and rubbing and the skin opens up, peels and scabs. I have no idea what it is. I've tried cortisone from over the counter to the absolute highest script offered...nothing clears or heals it. Now that foot has three spots, ankle, top left and top of the big toe bone. I try heavy moisture, albolene or Vaseline and cover it with a sock overnight & all day. Moisture seems to help....but temporarily. Dermatologist has just provided topical cortisone. I am wondering what could be causing this...do I need to see a podiatrist? There was a period of time when the foot itself had serious discomfort along the big toe and the area under the toes (ball of the foot). Appreciate any assistance or direction.
