I Have Had Chronic Pain In My Back/leg/hip Since 1990

I have had chronic pain in my back/leg/hip since 1990 when I was injured from horseback riding - felt like my left leg was shorter than the right after the accident. I have been going to the chiropractor for about 20 years after shooting pain and suggestion from primary care physician. I am now 44 and very confused about what to do.

The MRI of my back shows L4-5 disc bulging and left annular fissure notable. Not sure what this means? The MRI of my left hip shows localized area of fraying involving the anterior labrum without more well-defined tear. What does this all mean, what would you pursue? Hip doctor wants to do surgery to stitch fraying. Not concerned with back.
