I Have Seen An ENT For Possible Vestibular Neuritis. This

I have seen an ENT for possible Vestibular neuritis. This was ruled out, it seems by a VNG test. I have been referred to a Neurologist. I'm trying to learn what things I should bring to the neurologist for my symptoms and past history, so that we can narrow the possible causes as soon as possible.
I am wondering if the fact that I had shingles about 4 1/2 years ago is pertinent. Also wondering if a swelling of my 2nd toe and severe inflamation is related? What other things should I bring up to the Dr? I have been in general good health all of my life. Non smoker, moderate drinking, generally clean lifestyle.
