I Self-referred To Mayo To Investigate Atrophy On The Right

I self-referred to Mayo to investigate atrophy on the right side of my body. I was diagnosed with myelopathy and with a small stable focus of chronic melomalacia or with a small syrinx at C3-4. Before I could confirm whether to do surgery, I began having headaches and having problems processing and losing words. I have sharp pains in my right ear which is now greatly enlarged and lower than the right ear. A brain MRI revealed multiple supratentorial FLAIR/T2 hyper intense lesions. I don't know where to turn--I'm uncomfortable moving ahead with a decompression surgery on the neck until I confirm the source of cranial atrophy and ear pain. Do you agree or have suggestions for where I can seek the best treatment?
