I Was Diagnosed With Low Vit D In February Earlier

I was diagnosed with low Vit D in February earlier this year. Prescribed a capsule of Vit D = 50,000 IU's 1 time a week for 12 weeks. At this point still feeling very fatigued, have the feeling of swollen/tight neck and something in throat when I swallow, hair loss, a bit of muscle/joint pain and the inability to lose weight, currently a good 40 lbs over weight. I'll back track 3 years ago to when I recieved 2 Lupron Depot injections to treat endometriosis. I believe the onset of fatigue and other sypmtoms followed the injections. Doctor in February ruled out hypothyroid and thus the diagnosis for low Vit D. Currently, sypmtoms of fatigue have increased to the point of a few low blood pressure or fainting spells. Decided to have lab work done to recheck Vit D level and thyroid. These are the areas of concern, especially the low MPV. What can you tell me about this? Should I be very concerned?
PLT at 457, a bit high
MPV at 8.5, a bit low
Folic Acid 5.4, a bit low
T3 Uptake 33
T4 9.3
T7 3.1
TSH 2.850
VitD 250H total 40.4
