I'm 66 Yrs. Have Sob Increasing In Last Few Months,

I'm 66 yrs. Have sob increasing in last few months, wt. 256 today from 265 couple weeks ago. Take Triam dly. Getting Phys. Therapy recently for both knee replacements recently, PT therapist noticed last few weeks sob increasing with very little activity. Have a cough, wheezing, heartbeat 115, Sao2 ok, 95-97. Having some PVC's no chest pain. Sleep on each side, not on my back. Had a sleep study recently, no sleep apnea. Extreme fatigue ongoing on for a few years minimum. Can't remember not being fatigued really but it's come to be extreme. Heart failure on each side of family, Dad, grandfathers. I'm getting more crabby & irritated lately. Also, I did have a CT scan for heart & lungs recently. Heart Dr. said every thing ok, even though I have 2 small nodes on left lung lobe. Have seen a Resp Dr. said maybe I have asthma, gave me a couple of inhalers. Not making a difference in breathing. With everything I'm reading, sounds like CHF? I live in the Midwest, may go to Mayo Clinic, local Doc's have no clue it seems to me. What say you? Thank you. Is a heart Dr. available?
