Is AT&T Advisable For Positive Result Of TB QuantiFERON?

I am a medical student suffering from back pain,weakness, fatigue and weight loss since last 6 months, lost about 7 kgs.chest radiograph normal.chest ct showed few subcentimeter mediastinal nodes.few subcentimater cervical lymph nodes on ultrasound.sputum negative. mri spine normal.tb quantiferon gold positive.pcr dna negative.should i use att? Which regimen

Hello,

Constitutional features of fatigue, weight loss with mediastinal nodes and positive TB quantiferon Gold standard indicates most presumptive diagnosis of tuberculosis. So far as regimen and drug dosages are concerned for the therapy of tuberculosis, it is determined by:
- Site of tuberculosis
- Weight of patient
- Drug susceptibility in lab data (either Rifampicin or multi drug resistant strains )
- Extent of disease with systemic involvement
- Clinician expertise

Duration may be that of 6 months to 1.5 - 2 years in case of CNS tuberculosis or abdomen Koch's. Type of treatment may be that of DOT (directly observed therapy) or home therapy. Usual drugs used for the control of tuberculosis are:
- Rifampicin
- Isoniazid
- Pyrizinamide
- Ethambutol
- With or without fluoroquinlones, streptomycin
