Is Alyacen Advisable For An Obese Woman With High Blood Pressure?
Hi, I m pediatrician. Patient 20 y/o with hx obesity 262 pounds, and HBP (161/98) no medications.Patient comes first time to the office for refill Alyacen birth control, but before started birth control pills presented high blood pressure, and obesity. I will plan to referral to cardiology for evaluation HBP, and I will order blood test for workup obesity. Patient could be continue in Alyacen birth control or is necessary to change another birth control pills. Thanks, Dr. Cruz

It is not safe to take Alyacen while suffering from High Blood Pressure (BP). The birth control pill Alyacen contains two hormones, a progestin, and an Estrogen.
It is thought that the Estrogen in the pill may trigger the release of other hormones that can cause the BP to rise. Hence, your patient should not continue with Alyacen medicine.
Consider a different form of birth control for her. Her BP may be less likely to increase if she takes an intrauterine device or birth control pill that contains a lower dose of Estrogen.
The hormone-free copper IUD, the hormonal IUDs which release the hormone Levonorgestrel (a type of progestin) or progestin-only pill appear to be safer and effective alternative contraceptive methods for someone with high BP.
In addition to this, she should be evaluated by the cardiologist to rule out other secondary causes of high blood pressure in this young lady.
