Is Ciprofloxacin Advisable When Diagnosed With Klebsiella

I have ankylosing spondiloarthropaty for 12 years. I am on methotrexate 25 mg weekly and azathioprine 250 mg daily.One month ago doctor told me that I had klebsiela UTI and gave me one sachet Urizone followed by Ciprofloxacin 1000 mg once daily for 7 days. three weeks later another test show that klebsiela is back. Now I got ciprofloxacin 500 mg twice a day for 5 days. Is that sufficient dose for immuno compromised person? many thanks.

Thanks for raising your concern on this platform and following is/are recommended guidelines:

1. Since Klebsiella organism are resistant to multiple antibiotics thus prior to starting any antibiotic therapy it will be wise to go for culture sensitivity test [CST] and once it is confirmed treatment may be modified.

2. Although klebsiella respond to ciprofloxacin but it seems that the bacteria is resistant to the antibiotic [reason of coming back] so CST,urinanalysis, and later cystoscopy is must to evaluate the sensitivity and if the symptoms continue further options can be discussed with your physician, till then:

. drink plenty of fluids
. no acidic/spicy food, alcohol
. avoid milk/milk products

3. As you are taking methotrexate and ciprofloxacin increases the toxic effects related with the drug thus remain under regular supervision/guidance of your doctor since ciprofloxacin decreases the ability of kidney to remove methotrexate from the body.

PS. You should tell your attending doctor about the simultaneous use of both the drugs, since then he may monitor the amount of methotrexate in blood or may change the dose or simply ciprofloxacin and see for jaundice,dark urine,easy bruising,black stool,fever and call him right away if it happens.

4. Relapsing infections may be due to structural abnormalities, abscesses, or other problems that may require surgery, and such conditions should be ruled out, so check with an Urologist also.

5. Infection that take advantage of weak immunity [immuno compromised] are called as opportunistic infection thus the dose and duration will depend on the type of infection you have.
