Hi.
Let us be bit systematic in thought process::
You say this problem is since April 2012- almost 2 years= so NO chances of cancer.
There are no known cancers of
epididymis.
Quantiferron is a reliable and sensitive test for TB.
IN TB of
epididymitis , other tests of CT , x-ray may be negative.
You can not have biopsy - very true if you are not married yet. Please undergo
semen analysis for
sperm count, quality of sperms if present, for TB bacilli ( AFB Staining ) TB culture if the facility is available.
FNAC is a good test and can help diagnosis, must be done under
ultrasonography guidance for accurate aspirate. But there is no guarantee of positive results as epididymis has a very atypical anatomical structure , which might have changed in your case.
Tuberculosis of epididymitis has a classical finding of ''beading appearance''. If this is there you can start anti-TB treatment.
another point is - one can always treat the patient on empirical basis for clinically suspected TB to see results and get treated at the same time.
Pain in abdomen can be due to TB lymph nodes / affection of the
spermatic cord which enters the abdomen through a canal and enters the prostate.
Request your Doctor to start anti TB treatment as I directed . You may get great results ( if not already started )
