Is Pamelor The Right Medicine For Chronic Cough?

my sister was just prescribed Pamelor for a chronic cough by ENT specialist. She is highly sensative to most drugs and has allergic reactions to all antibiotics except for 3..in the errythromycin family, I m unsure why this antidepressent was ordered for her chronic cough. She had respiratoryinfection past 3 wks with rx of antibiotics. This cough has persisted for years, 1st said to have asthma, then COPD, she coughs as if she has an airwAY obstruction, which I had with vocal cord paralysis and use of trach for 14 yrs. I am an RN and usually attend my sisters MD visits as she has a mild retardation, however she up and went to our known ENT specialist on her own thinking he would clear her sinus . As its Friday I need to wait till Monday to call Dr. for answers but am concernedfor weekend as she is bent on using med. I know this drug inhibits histamines & acetycholines and am wondering if this reason for use? Thank you for your time and input. Sorry, please cancel question, I was unaware there was a fee.

Anesthesiologist 's  Response
Hello,

According to the mentioned symptoms, I suspect a pulmonary pathology like asthma, COPD, allergic bronchitis, eosinophilia. Achest physician can arrive at an accurate diagnosis after clinical examination and required investigations. One thing I can certainly tell is that Pamelor is in no way helpful for treating chronic cough. To those patients who visit my clinic with such symptoms, I do order the following investigations only after a thorough and detailed clinical examination:
1) chest x-ray PA view
2) sputum examination
3) absolute eosinophils count
4) pulmonary function tests.

As you had been to so many specialists, I can understand your pain. Just once see a chest physician once and discuss the above issues. Now I can suggest few medications that would provide her a temporary relief:
1) if it is a productive cough, expectorant syrups containing bromhexine, guaiphenesin can be used
2) for dry cough, antitussive codeine containing syrup is useful 3) a bronchodilator short-acting beta 2 agonist spray inhalation would help in relieving shortness of breath.
4) exposure to trigger factors like cold environment, dust etc to be avoided.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Pallavi.m,
