Is A Raised, Hard Black Mole Be Of Concern?

My son was born with a black mole in the middle of his forehead like the Bindi worn by many Indian women. Today he complained that it hurts when he touches it. In 11 years, I have never felt it to be hard, but now it is a little more raised and a bit hard. What might be happening?
