Hello! well that's a result of chronic ear infection. I always recommend to patients with cholestoma to remove it surgically. But before the surgery, it is advised to have a 'dry ear'. That means that the ear should be free from an active infection. I highly recommend to consult an ENT specialist to get the cholestoma removed. The doctors will advice antibiotics for at least one week and then go for the surgery.
