Is Hysterectomy Advisable For Endometrium Thickening?

I am 53 years old with post-menopausal bleeding. I had a biopsy last month, which was negative. I started bleeding again. I went to another doctor for a second opinion and just learned that my endometrial lining is 9mm thick. I am scheduled for a D&C next Tuesday. Should I just ask the doctor to perform a hysterectomy so I can be proactive? I don t want to take any chances with cancer.

Hello,

Usually in postmenopausal women with bleeding and with endometrial thickness of more than 5mm is considered for biopsy. As your biopsy came to be negative, there is very minimal or no risk of cancer.

Considering post menopause bleeding, it's better to undergo D and C first and see if the bleeding controls. The following are few indications of hysterectomy in postmenopausal women:
1) High risk of cancer
2) Abnormal uterine bleeding that is not controlled conservatively
3) Biopsy positive for carcinoma
4) Multiple uterine fibroids causing severe pain and bleeding
5) Uterine prolapse
6) Endometriosis etc.

As your endometrial thickness is 9mm which is well above 5mm, hysterectomy can be considered provided there is no improvement with D and C.
