First of all, it is pleasing to know that you got much relief in your pain with the ibruprofen. But since you have suffered from acute gouty attack so using allopurinal is not recommended in the acutre attacks because it can wporsen the symptoms. Allopurinal is used as a prophylaxis for the occurrence of such attacks in future. Also that it takes 2-3 months to start working at its full swing. However, Anti inflammatory agents like
Ibuprofen are beneficial in decreasing the swelling as well as pain. Normally Allopurinal may be started week or two later after settlement of an acute attack.
Secondly since you are using quite high dosages of ibuprofen, so you can develop acid regurges or Drug induced
gastritis, so if you had been my patient i would have suggested you to start using
Omeprazole 20mg before breakfast daily for two weeks along with continuing ibuprofen for two weeks.
Ibuprofen can also be used with allopurinal for fist four weeks in low doses to avoid occurrence of acute gout attack.
Thirdly, i would like to suggest you to take
Vitamin C supplements, and daily 1000mg of Vitamin C is shown to decrease chances of gout attacks quite convincingly.
Lastly, you have sought to look for the reason for having such attack. Its is multi factorial. Many risk factors like
diabetes,
Hypertension, Obesitry, Sedentary life style, Immobility, Increased fructose or sugar beverages, Alcohol, pulses and use of diuretic drugs increase its incidence, but lack of data regarding your age, your life style and other comorbids exist, so that would be difficult to tell that what caused such attack in your case. Also that, many people are naturally low excretors of uric acid naturally so it is common in them. Men are found to be more vulnerable. On the contrary, Healthy and balanced diet, exercise,
weight loss, Life style modification, decreasing high sugar drinks and alcohol and more fiber diets along with diets rich in Vitamin C are found to decrease its incidence.
