Is It Safe To Take Diltiazem Instead Of Metoprolol Despite Suffering From Atopic Dermatitis?

My husbands cardiologist changed my husbands medication today via a phone call I made to him today because my husband s skin was burning too much with ultra light treatments he gets for atopic dermatitis. He s been getting ultra light treatment over a year. In March his skin started burning more than usual. The last 6 weeks his whole body has been burning - he runs a fever (low grade) after his weekly treatment, has chills and chattering teeth. His skin flakes and is itchy for the entire week. The only other medication that my husband takes(besides eye drops for macular degeneration) - is metoprolol which he has been taking since May 2014 (25 mg), but in March 2015 his doctor increased the dose to a tablet in the AM and PM. He takes metoprolol for A-Fib with unknown causes because he can not take blood thinners due to a bleeding problem. Several weeks after the increase of metoprolol my husband s skin started to burn and peel. He isn t sleeping well - is always cold - and has vivid dreams. His cardiologist has just prescribed a new medication for him - Diltiazem. He wants my husband to take metoprolol once a day until he can be weaned off in addition to diltiazem 24 hr ER 180 mg Tablet. I just looked up the side effects for diltiazem and found MANY side effects under Dermatology. It s kind of scary thinking of my husband taking a medication that may cause dermatitis (or any other skin problems) when the purpose of the ultra light treatments is to control hid dermatitis. His dermatitis does not want to stop the ultra light treatments. And, since this a holiday weekend, I doubt that we can get in touch with either of his doctors. I am wondering if my husband should hold of on taking the diltiazem until he can talk with his dermatologist next week. I think that maybe both doctors should talk with each other. What do you think?

Anesthesiologist 's  Response
Hello,

Your suspicion is right and diltiazem can cause
Photosensitivity
Toxic epidermal necrolysis

So it is better to withhold the drug until you notify your dermatologist about the medication. If your dermatologist says it's fine to use, only then you can proceed to use it. Please consult your skin doctor as soon as possible. Also, notify your cardiologist.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Dr. Pallavi M., Anesthesiologist
