Is Pain While Sitting And Standing Suggestive Of Incomplete Abortion?

I am a 37 year old mother of a 13 year old boy. I suffered a miscarriage in November at 21 weeks due to cervix thinning and infection, had a cerclage and then D&E. On Wednesday, pregnant again at about 6 weeks, I had a scan to determine the viability of the pregnancy, and was told that gestational sac had shape and that the fetal, pole, yolk sac had not gotten bigger, but a little smaller and that I was miscarrying. Wednesday evening I had sex which was a bit painful so I stopped and a little bleeding that went away. Yesterday evening I started to have severe pain in my abdomen and lower back; it felt like contractions every few minutes. I bled but only when I went to the bathroom on the toilet, maybe from bearing down, I m not sure. Whenever I went to urinate, the bleeding was minimal, very light, a few drops to what you get on the first day or your period, when but I passed two clots (no blood in my urine). After a couple of hours it subsided, pain was far less strong and I was able to sleep and bleeding is spotty. I never went to the hospital, I stayed home. Today I went to the dr and she did a transvaginal sonogram and said that there was nothing in my uterus, that I had passed the pregnancy. She did a vaginal exam as well, which hurt a bit and said that my cervix was closed. My concern is that the bleeding was so minimal. I can t see how I could have passed it, especially with her saying my cervix was closed. Would it have closed that quickly? Also, my normal menstrual cycle is like 3x more. i am also still feeling some pain especially when I sit down. Is it possible that it hasn t passed completely? And, can you tell me if it s normal to have this pain when sitting or standing? I feel like there is more in there that hasn t come out. Thanks so much for your help.

General Surgeon 's  Response
Hello,

As per my clinical experience, the subjective feeling of lower abdomen heaviness is related to uterine swelling or stretching post abortion episode. As the trans-vaginal ultrasound has confirmed empty uterus, it is final verdict and not to be challenged on subjective feeling grounds.

In my clinic, I do recommend a TORCH panel evaluation on mother side for finding out the reason of recurrent pregnancy loss.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
